If you would like to request an absentee ballot to be mailed to you, please complete this form and mail it to:
Village of Cottage Grove
221 E Cottage Grove Road
Cottage Grove, WI 53527

Once received and you have completed your ballot, mail back in the envelope provided with the required signatures. These returned ballots must be returned by 8:00 p.m. on election day.

The U.S. Supreme Court has affirmed the photo ID provisions of 2011 Wisconsin Act 23.

A photo ID is now required to receive a ballot in all special and regular elections.
